1. Get a Physical 

• Physicals must be completed with the physical form, which can be downloaded here --> (PDF)
  • Physicals are provided on site by St. Luke's Medical Group and scheduled by the athletic training staff for each individual sport program. 
  • Physicals on all other forms will not be accepted
• Physicals must be done by an MD, DO, NP, or PA
  • Physicals done by a chiropractor, physician assistant, or any other medical professional will not be accepted.
• Completed physical forms should be uploaded to Healthy Roster

2. Make a Healthy Roster account 

• Information will be sent to your mobap.edu email account
• Provide your information to the Sports Medicine Staff --> New Student-Athlete Information
• Follow these INSTRUCTIONS.

3. Complete the forms and upload the following information to Healthy Roster

• Complete Medical History
• Complete Pre-Participation Form
• Upload the front and back of your insurance card
• Upload physical
